UNCC works to improve access to health and wellness services for vulnerable individuals and families across Brisbane. Through coordinated and integrated care, we ensure people receive the right support at the right time — promoting dignity, recovery, and wellbeing.

At UNCC, we believe that good health is the foundation for a stable and fulfilling life. Our Inclusive Health Partnerships aim to improve access to essential healthcare and wellness services for vulnerable people across Brisbane.


Hospital Partnerships

We partner with major Brisbane hospitals and healthcare providers to ensure coordinated support for individuals with complex medical and social needs who frequently present to emergency departments. Through collaboration and follow-up care, we help clients access the ongoing health, housing, and community support they need to recover and rebuild stability.


Care Finder and Aged Care Support

In partnership with the Department of Health, UNCC provides additional support for older people who face challenges navigating My Aged Care or managing their health needs. Our goal is to ensure that no senior “falls through the cracks.” We help connect older clients with the right aged care, medical, and community services — promoting dignity, safety, and wellbeing in later life.
